Immunizations

Texas law requires students attending public schools to meet state immunization requirements or have a valid exemption on file.

Students must provide acceptable evidence of vaccination prior to entry, attendance, or transfer to Liberty Hill ISD. Campus health services staff can review your student's immunization record, identify any missing requirements, and provide information about vaccination resources.

Texas Minimum Immunization Requirements

Immunization requirements for Texas schools are established by the Texas Department of State Health Services (DSHS).

Families are encouraged to review the current requirements before enrollment and at the beginning of each school year.

Immunization Records

Immunization records must include the student's identifying information and the month, day, and year each required vaccine was administered.

Acceptable documentation may include:

A record validated by a physician or public health provider.
An immunization record generated from an electronic health record that contains the required provider and clinic information.
An official record from a state or local health authority, including an applicable ImmTrac2 record.
An official immunization record provided by another school or school district.

Please provide updated immunization documentation to your student's campus nurse.

Immunization Exemptions

Texas law provides certain exemptions from school immunization requirements. Families seeking an exemption should follow the requirements established by the Texas Department of State Health Services.

Reasons of Conscience

A parent or legal guardian, or a student 18 years of age or older, may claim an exemption for reasons of conscience, including a religious belief, by submitting the required DSHS affidavit.

The affidavit must be completed, signed, notarized, and submitted to the student's school. A conscientious-exemption affidavit is valid for two years from the date of notarization.

Completed original exemption documentation should be provided to the student's campus nurse.

Medical Exemptions

Texas law also provides for medical exemptions when vaccination would pose a significant risk to a student's health.

A medical exemption requires a written statement from a physician licensed to practice medicine in the United States who has examined the student. The statement must identify the vaccine or vaccines for which an exemption is medically necessary.

Unless the physician states that the medical condition is lifelong, the medical exemption is valid for one year from the date it is signed.

Provisional Enrollment

Under certain circumstances, Texas law allows a student to be provisionally enrolled while completing a required vaccine series.

A student may qualify for provisional enrollment when the student's immunization record shows that the student has received at least one dose of each required age-appropriate vaccine and is not overdue for the next required dose.

To remain provisionally enrolled, the student must receive subsequent required doses as soon as medically feasible and provide acceptable documentation to the school.

Students who do not remain in compliance with the required vaccination schedule may be excluded from school attendance until the applicable requirements are met.
